Whel cant be off centre as the spline is keyed (a couple of splines missing) so the wheel can only go back on correctly, unless of course forced on....

The indicator in cancelled by two small lugs in the clockspring mechanism and if the indicator stalk is not siting in correctly the lugs wont trip off the canceller in the stalk. It is also possible the small cancelling lug on the indicator has been broken, though if it cancels in one direction maybe not.
