We've had temperatures here as low as -8*C, mine cranks a tad longer after cold soaking for a night. On mine the diagnostic does not log anything when cranking.

A few weeks before I replaced the hose from the low-pressure pump to the high-pressure pump and did not prime it, it would crank longer randomly, until I opened the prime port with the engine on, to fill that section of pipe with diesel

Usually surging when cold is because of a bad pressure regulator.

Surge can also occur with an injector that has too high leak-off. When that happens you can also feel the exhaust pulsing ad idle.

Sorry, but life ceases to exist here if we get below +8*C let alone -8*C!

Excellent tip, thanks. After having the system open, I primed it just by cranking until it fired (while watching the pressure climb). There's every chance I still have a bit of air in the bleed branch/valve.

I have not checked. Will do this. I'll also do a leak-off test this weekend. I suspect I have a dead injector because it has stared to run rough under load but gets smooth once it warms up (new behaviour in the last few days).

I'm booked for the professional diagnostics in a bit over a week with a local mechanic who has the DRB equipment and took less than two sentences to totally convince me that he knows these engines.

Interestingly, another diesel specialist I rang said:
"I'll sell you the injectors but I won't fit them. The day I have it booked in at least half my blokes will call in sick for sure. Getting to 4 & 5 really sucks and I'm not keen in cutting holes in your panels." Clearly he also knew his stuff also.
What he did offer me was to test all five injectors for $95 each, pick the best three and sell me two new ones for 4 & 5 for $443 each. Not a bad offer.

That's the wrong injector. The correct one is $375:

Either a good result, or I have four stuffed injectors.

How long did you run the test for?

What is the volume of fluid from one of the four that are the same?

The volumes are pretty consistent which is usually a pretty reasonable sign. If it's a high volume in a short time, this isn't so great though.

They are 100ml bottles. I didn't time the test. I'd guess about 5 or 10 minutes.

I was more looking for a difference than a leakage rate. I was being factious about having 4 stuffed injectors. If 4 had excess bypass leakage then it wouldn't reach rail start pressure.

I've also vacuum primed the fuel system and tightened the clamps on all the rubber hoses. It starts better and I've not noticed any more rough running. I suspect a tiny air leak.
