I believe its the 4864 046 WASHER Thrust number 21 on the list. Has anyone fixed that rattle noise?

Yes (number 21 ) is the thrust washer and it holds the crank shaft in position so it doesn't slide back and forth to much (i think maximum movement is 0.3 mm) but mine moves about 1.5mm due to thrust washer being worn .
Tried to get new one but could not find it anywhere so just put the old one back. It wares out due to riding the clutch while driving mostly beside the normal ware .
Can't say that there any unusual noise on my car due to that thrust washer but the linkage to the transfer case does make a rattling noise at low rpm especially when taking of from standing still .

I had the same issue after I changed the clutch. Mechanics told me that the new clutch would just be amplifying an existing issue in the gearbox. Drove around with it like that for ages and just held my clutch all the time until I went to see a transmission specialist in Christchurch. The guy there left it running in neutral and moved the gear lever towards a gear (without the clutch pushed in) and the noise pretty much went away. He then asked if I had changed the clutch recently and had put a petrol clutch in. After confessing to the crime I forked out for a proper diesel clutch and sure enough, the rattle was gone. Something to do with the dampening effect of the springs in the clutch disc.

So if your problem started after a clutch replacement...
