I've rebuilt the steering gear on my Tj before and comparing it to the pics in the FSM it's not just the casting that's mirrored. The internals are mirrored too. The splines on the pitman shaft are offset to the other side as well so RHD and LHD pitman arms are also different.
I've read that some US vehicles use a steering gear outside the frame rails, which may be the same as RHD boxes mounted inside the frame, but I've never found which vehicles were like that.
