My offset joint was fitted by Narellan Truck Align. They sourced it from within Oz, somewhere in QLD, from Safety Steer. Pedders also said they could supply one.
The joint replaced on mine was the near side upper joint. A 1.5 degree was used and it was adjusted to give the maximum 1.5 degrees. It now sits perfectly on the road.
Saying that, last week I did a deep water crossing and lubed all joints afterwards. when I pumped grease into the new upper joint the top cracked off it! It is being replaced under warranty, but I hope it was just one dodgy batch and not an inherant fault in the brand.
The small ring (looks like a washer) in the second pic is actually part of the machined housing and holds the top plate (the piece with the nipple) in position. It cracked at the rear of the joint, and then the crack slowly worked its way around until the whole thing lifted off.
