Good you ran injector cleaner,the t/o has to do with the throttle (t being for throttle, o is your guess)
There is a couple of O2 sensors before the cat you might want to
Look at, make sure you are getting good connection.
Are you getting any misfire?

This is interesting.
My XJ does not have a O2 sensor or a cat.
The 2.5 turbo was not sold in the USA so there is very little info about them on NAXJA.
Has the injector pump been worked on in any way? eg seal replacement? If so the body may have been moved a little. You could try moving the body back about 1/10 mm . Be careful though as this can cause high idle & rough running.
Does it idle normally when it has not been revved ? If you slow the revs gradually does it settle to a normal idle?
What is its idle speed?
Does it start to loose power when it is worked hard for a while, like accelerating to pass a line of trucks or accelerating up a steep hill?
My initial thoughts are a fuel delivery problem.
Best only take notice of people who have a VM diesel with electronic throttle.
There is a good VM diesel group on yahoo base in the OP's part of the world.
The motor was used in several different applications in Europe.
