The big suggestion is to stop filling up at discount fuel outlets like Costco and use BP Ultimate diesel. So I'm doing that and adding a fuel conditioner each time I fill up at a rate of 1ml per L. I'm on my second tank, I didn't notice any improvement on my first tank. I'll check back in after the next tank.
SHM were sayng that they've had a number of issues with other diesel jeeps that got fuel at Costco and a fresh tank of BP, Ampol or Shell sorted them out.
We'll see.
I would say the issue with discount servo's is not so much the fuel as any money it comes from the same refinery, but the fact the tanks prob get filled more often and the fuel doesn't get a chance to settle after being filled and shit is still floating about in the storage tanks .

Hence why you should never fill up when the tanker is there, especially with our tight tolerance diesels. Plenty of condensation/water floating around the tank when it is being filled.
